Taylor Lemke, MSW, LGSW, is dedicated to providing emotional support through a strengths-based approach. Taylor works collaboratively with clients to set realistic and attainable goals, focusing on self-care practices and addressing barriers to physical and mental well-being. Taylor has a special interest in supporting individuals who face mental health challenges due to chronic illnesses, particularly type 1 diabetes. Whether clients are newly diagnosed or dealing with diabetes burnout, Taylor offers guidance through these transitions and struggles.

We are in network with Medical Assistance, Blue Cross Blue Shield, Health Partners, UCare, Optum and Cigna. If you have questions about coverage, please contact our office.

Sliding fee program

A sliding fee discount program for people with limited financial means, whether uninsured or underinsured. Eligibility is based on the Federal Poverty Guidelines.

If you are not using in-network insurance, you can request a Good Faith Estimate of your expected cost before scheduling any service.
