The program is designed for those with limited financial means, whether uninsured or underinsured. Eligibility is based on the Federal Poverty Guidelines.

How do I apply?

Does it discriminate?

Good Faith Estimates

The Good Faith Estimate (GFE) is a requirement under the No Surprises Act (2022). It provides a written estimate of the total expected cost of your treatment if you are not using your in-network insurance benefits.

What does it include?

The GFE includes all related costs, such as medical tests, prescription drugs, equipment and hospital fees.

When will I receive it?

You should receive your GFE at least one business day before your scheduled service. You can also request a GFE before scheduling any service.

What if my bill exceeds the estimate?

If you receive a bill that is at least $400 more than your GFE, you have the right to dispute the bill. Keep a copy or picture of your GFE for reference.

For more information about your rights, visit cms.gov/nosurprises or call 800-985-3059.
